How To Send An Instrument To Sklar For Repair?
There is no authorization number required. Simply send the item(s) to:

Clean and sterilize instruments before returning.
Carefully pack instruments to prevent damage during shipping.
Include a description of the problem, along with a copy of your Purchase Order.
If a repair estimate is required, please notify us in writing. Be sure to include your name and contact information (phone, e-mail, etc.). Repair services will not be conducted until approval is received.
Please provide an address for return shipment (no P.O. boxes). Shipping insurance in excess of $100 must be requested in writing.
